Points to note

  • Episodic disease appointment could be made by phone/ mobile applicaiton on a “first-come, first-served” basis.
  • Only one appointment would be arranged via each telephone call.
  • Service will normally not be provided to patients who are late for more than 30 minutesNote 1.
  • Each eligible patient could, on a need basis, keep one episodic disease appointment (either general consultation or acupuncture) and one follow-up appointment (either general consultation or acupuncture) at any one time.  A simultaneous second episodic disease/follow-up appointment at another CSCMC will not be entertained.
  • In case a patient could not attend the appointment as scheduled, he/she should cancel his/her appointment as soon as possible so as to release the disc to others in need.
  • When civil service eligible persons (CSEPs) arrive at the CSCMCs, they must present original proof of identityNote 2 (e.g. Hong Kong Identity Card (HKID), Hong Kong Birth Certificate (HKBC)) for eligibility checking at the CSCMCs registration counter. For CSEPs who do not hold HKID/HKBC, they must provide their date of birth and the HKID number of their associated eligible persons for the CSCMCs to verify their eligibility for civil service medical benefits.
  • For CSEPs whose personal particulars have not been included in the Medical and Dental Benefits Eligibility Checking System, they must produce a valid G.F. 181 (Rev. 2008)/ Try. 447 (Rev. 2008)/ LS. 181 (Rev. 2008) as appropriate for verification of eligibility when visiting the CSCMCs.
  • Subject to the operation of the CSCMCs, the above booking arrangements may be adjusted to achieve a more effective use of the appointment slots and to streamline the operation of the clinics.  The revised arrangements, if any, will be announced in advance as appropriate.

Note 1: CSCMCs may exercise discretion in considering whether CM services would be provided to the late-comers taking into account the actual circumstances.
Note 2: Proof of identity refers to the documents mentioned under Section 17B of "Immigration Ordinance" (Cap 115) of Hong Kong Law.
