TWO FORMS IN ONE

Civil service eligible persons* seeking reimbursement or direct payment of expenses from the Department of Health may now use one form for either purpose instead of two forms for separate purpose.

At present, applications are made to the Director of Health for reimbursement of expenses for drugs, equipment and services - which form an essential part of the medical treatment - as prescribed by the Hospital Authority doctors on medical grounds but are not available in the hospitals/clinics concerned or are chargeable by the Hospital Authority. The arrangements, including the application form, are set out in CSB Circular No. 14/96.

Separately, the Hospital Authority has put in place direct payment arrangements for percutaneous transluminal coronary angioplasty and intraocular lens, purchased by the Hospital Authority on behalf of patients. Civil service eligible persons may complete a direct payment application form for the Hospital Authority to arrange for payment settlement with the Department of Health direct.

For administrative simplicity, application forms for reimbursement and direct payment of expenses have been consolidated into one. It is more user-friendly and it also facilitates the attending doctor, in making the written confirmation, that the drugs, equipment and services are essential on medical grounds.

This form is attached to CSB Circular Memorandum No. 8/2002 issued on April 30, 2002. It is available from the Pensioners' Welfare Unit, Civil Service Bureau and the Pensions Division, Treasury. * Civil service eligible persons include civil servants, retired civil servants in receipt of a pension or an annual allowance and their dependents (spouse and unmarried children under 19 or under 21 who are in full time education or otherwise dependent on the officer as a result of physical or mental infirmity).
